Level Upgrades

For officials wishing to upgrade from Association level to National level, or to upgrade from National level to Master level official. 

Completed forms should be scanned & e-mailed to SWTFOA Certification Chair Jim Carlisle at jim.carlisle@verizon.net. To mail it to him, e-mail Jim for his address so he will be expecting it.
